MSDH offering one-stop shop event for back-to-school forms
SUMMARY: The Mississippi State Department of Health is offering a one-stop shop for required school entry/registration forms. From July 24 to August 4, parents can go to the MSDH’s Office of Vital Records in Ridgeland to get their child’s immunization form and birth certificate. These documents are required before a child can enter school. The Form 121 will be available for free during the event, and a new birth certificate will cost $17. Mississippi state law mandates that children be immunized against childhood diseases to attend school, but religious exemptions are now allowed.
